Select the type of UnionBank Credit Card most applicable to you at

STEP 1: Fill out the form with your Contact Details. Read the Terms and Conditions and tick the check box to agree. You also need to check the tick box for reCAPTCHA for human verification. Proceed by clicking Submit.

STEP 2: A pop-up screen will appear about the Declaration Agreement. Scroll down and read the entire document. Click ACCEPT if you agree.

STEP 3: Fill out the form with your complete Personal Details and click Submit.

STEP 4: Fill out the form with your Work Details and click Submit.

STEP 5: If you wish to add other card details just click on the green add button on the upper right. Otherwise, click Submit.

STEP 6: Upload the required documents such as your valid ID and proof of income. Click Submit to proceed to the last step.

STEP 7: After checking that all your information is correct, you may click Finish on the lowermost part of this page.

That's it! An email regarding your application will be sent to your registered email address.
