STEP 1: Fill out the form with your Contact Details. Read the Terms and Conditions and tick the check box to agree. You also need to check the tick box for reCAPTCHA for human verification then proceed to the next step by clicking NEXT.

STEP 2: A pop-up screen will appear which is the Declaration Agreement. Scroll down and read the entire document. Click ACCEPT if you agree.

STEP 3: Fill out the form with your complete Personal Details and click CONTINUE.

STEP 4: Fill out the form with your Work Details, upload a picture of your Valid I.D., and click CONTINUE.

STEP 5: After checking that all your information is correct, you may click CONTINUE to accomplish the application.

That's it! You may then visit any UnionBank branch to claim your card and don't forget to bring your initial deposit.
